Welcome to Your Patient Centered Medical Home!

Our team of physicians, physician assistants and clinical staff works together to provide high quality care for our patients. We coordinate all of your medical care and treatment to ensure that you receive the necessary care when you need it. 

When you are sick or injured (not work-related or life threatening), call our office first. If you have acute, sudden onset symptoms, we offer same day office and telehealth appointments. When you are healthy, we remind you when it’s time for your annual physical or preventative test such as colonoscopies, mammograms, etc. If you have an after hours medical issue that is non-life-threatening but may require prompt attention, our after hours call service will notify one of our providers. If you have an illness, injury or condition that requires a specialist our team will coordinate your care with a specialist that we know and trust.

Open Access Care

It is very important to us that we are able to provide same day care for injured or sick patients, and we always try to leave enough space to allow us to see patients in our schedule who need immediate care or consultations.  This allows us to provide better care for you because you are able to see a provider from your doctor’s team with your medical records readily available.  However, because of limited scheduling space, if you need a same day appointment we strongly encourage you to call as early in the day as possible.

Please bring the completed forms to your first visit, along with your driver’s license and insurance card. You can also come to your appointment 30 minutes early to complete the forms, the staff at the front desk will have these forms available for you.

Financial Policy

We require all patients to present their insurance cards at each visit. If your insurance changes you should contact our office and your insurance company prior to your next visit to make sure you are covered. 

Payment of copays and outstanding balances is expected at the time of service. We accept cash, checks, Mastercard, Visa and Discover.  

No Show/Cancellation Policy

We require a 24 hour notice for appointment cancellations. After three “no shows” within a year the patient and their family will be discharged from the practice.

Additionally, if you need a referral, we work closely with specialists who we know and trust.

Financial Policy

Please bring your insurance information to each visit, and remember that payment is expected at the time of service unless prior arrangements have been made. There is a $10 statement fee for any co-pay that is not paid at the time of service, and we accept cash, checks, money orders, Visa, or Mastercard. Health insurance is a contract between you and your carrier, and it is important that you understand the coverage that it provides. West Michigan Family Health works with most health plans and will be happy to courtesy bill all others. If you do not have any insurance coverage, we also offer payment plans for established patients.

Twenty-Four Hour Coverage

West Michigan Family Health always has a doctor that is available twenty-four hours a day, every day of the year, but we encourage you to address routine questions during office hours so that emergencies can be prioritized. We also utilize an answering service for after-hours emergencies, but if this service is used excessively or inappropriately a $20 charge per call will be assessed.

No Show Policy

West Michigan Family Health requires a 24 hour notice for appointment cancellations, as timely cancellations allow us to accommodate other patients’ needs.  Additionally, there is a $50 fee for missed appointments; if there are three “no shows” the patient and their family will be discharged from the practice.
