Welcome! Thank you for choosing West Michigan Family Health as your Healthcare Provider
What to bring and expect at your first visit:
- Bring Insurance card and ID.
- Bring any current medications in the bottle or a list with name, dosage, and frequency.
- New Patient Paperwork. Either print and complete ahead of time or arrive 25 minutes early to complete required paperwork.
- List/Names of any specialists or previous providers that you would like us to request records from.
Open Access Care
It is very important to us that we are able to provide same day care for injured or sick patients. We always have same day appointments available for our patients who need immediate care or consultations. This allows us to provide better care for our patients because you are able to see a provider from your doctor’s team with your medical records readily available. However, because of limited scheduling space, if you need a same day appointment, we strongly encourage you to call as early in the day as possible.
Office Policies
We require all patients to present their insurance card and ID at each visit. This is to make sure that we have the most accurate information on file at all times. If your insurance changes you should contact our office prior to your next visit to make sure you are covered and that we are in network with your new insurance. Payment of copays and outstanding balances are due at the time of service. If you are ever in a pinch, please let us know, we do have payment plans available.
No Show-Cancelation Policy
We require a 24 hour notice for appointment cancellations. After three “no shows” or same day cancelations within a year, the patient and their family are at risk of being discharged from the practice.
Care for Minors
Children under the age of 18 years of age are considered minors. Minors are required to be accompanied by a parent or legal guardian. A written consent to treat form must be signed and dated in person by a parent or legal guardian if the child is unaccompanied or brought by another trusted adult for the appointment. The parent or legal guardian accompanying a minor for services is responsible for any required payments at the time of service (i.e: copay, insurance, balances on the account).

We work with our patients in sickness and health to help them not only feel their best, but to stay that way through current preventative recommendations. Additionally, if you need a referral, we work closely with specialists who we know and trust.

Financial Policy
Please bring your insurance information to each visit, and remember that payment is expected at the time of service unless prior arrangements have been made. There is a $10 statement fee for any co-pay that is not paid at the time of service, and we accept cash, checks, money orders, Visa, or Mastercard. Health insurance is a contract between you and your carrier, and it is important that you understand the coverage that it provides. West Michigan Family Health works with most health plans and will be happy to courtesy bill all others. If you do not have any insurance coverage, we also offer payment plans for established patients.
Twenty-Four Hour Coverage
West Michigan Family Health always has a doctor that is available twenty-four hours a day, every day of the year, but we encourage you to address routine questions during office hours so that emergencies can be prioritized. We also utilize an answering service for after-hours emergencies, but if this service is used excessively or inappropriately a $20 charge per call will be assessed.
No Show Policy
West Michigan Family Health requires a 24 hour notice for appointment cancellations, as timely cancellations allow us to accommodate other patients’ needs. Additionally, there is a $50 fee for missed appointments; if there are three “no shows” the patient and their family will be discharged from the practice.
